$index_format by default uses the %{} expando: %{%b %d}.  This uses the 
locale setting but you can prefix it with a ! to use C, e.g. %{!%b %d}.

$date_format is used for the %d and %D expandos in $index_format, and 
the %D expando in $folder_format.
