Thanks.  That was helpful.

The issue was the smtp_url.  In the debug output I saw the line

    External authentication name: jeff

But that misses the user's domain name.  So I needed to change

    set smtp_url =  "smtp://jeff@MY_MX_HOST:587"

to

    set smtp_url =  "smtp://jeff@p27.eu@MY_MX_HOST:587"

Indeed, adding the password helped as well.

    set smtp_url =  "smtp://jeff@p27.eu:SECRET_password@MY_MX_HOST:587"
